Medicare Insurance Information Requirements for Lymphedema Garments

Under the new Lymphedema Treatment Act, Abilico offers this unofficial guide to help healthcare providers and patients understand the documents required for insurance coverage of compression treatment items. This document list is subject to change as we receive new information and does not guarantee coverage. It is intended to assist in qualifying a patient for coverage.

Required Documentation for Beneficiaries:

Beneficiary’s Demographics and Insurance Information:

• Option 1:

  • A face sheet that includes the patient’s full demographics, date of birth (DOB), and detailed insurance information. This should encompass insurance ID numbers, the full name of the insurance plan, and the insurance company's contact phone number.

• Option 2:

  • A photocopy of both the front and back of the patient's insurance card(s).

  • The beneficiary's contact information, including current residential address and DOB.
